Community Care
At Home Healthcare offers programs that are administered and referred by
the Texas Department of Aging and Disability Services. The eligibility
requirements for these programs, both financial and medical, are
determined by the Texas Department
of Aging and Disability Services
and Texas Medicaid Healthcare Partners. The following programs
are provided by At Home Healthcare:
Community Based Alternatives (CBA )
The CBA program provides in-home assistance to qualified individuals. These services may include:
- Attendants to assist with personal care needs and light household chores
- Skilled nursing services
- Medical supplies and equipment
- Minor home modifications
A physician must certify that an individual requires these services based on medical need.
Primary Home Care (PHC)
This program provides in-home attendants to assist with personal care tasks, including:
- Bathing and grooming
- Light household chores such as cleaning, laundry and meal preparation.
The PHC program does not provide skilled nursing services. A physician must certify that an individual's medical condition warrants the need for these services.
Family Care (FC)
Family Care also provides in-home attendants to assist with light household chores and personal care tasks if needed. These may include:
- Light-duty cleaning
- Laundry
- Meal preparation
- Shopping
The FC program does not provide skilled nursing services and does not require certification by a physician.
