Skilled Nursing Services

Our Skilled Nursing Services are led by Registered Nurses dedicated to meeting the needs of our patients.  Care is coordinated with the patient’s physician and is provided at home—our patient’s home, their family’s home or an assisted living facility.

Services provided:

  • Vital Signs & Glucose Monitoring
  • Disease Management
  • Catheter Care
  • Medication Teaching
  • Administration of Injections
  • Laboratory Testing
  • Pain Management
  • Infusion Therapy
  • Post-Surgical Care
  • Wound Care

Home Care Aides work under the direct supervision of a Registered Nurse to assist patients with bathing, dressing, grooming, and other personal care needs.

Medical Social Workers are on staff to consult with patients and their families on coordinating community resources, financial assistance, and family counseling.

At Home HealthcareRehabilitation Services

Our Rehabilitation Services allow for continued medical treatment in a comfortable, familiar environment.  Our Therapists work with physicians to develop a plan that will meet the rehabilitative needs of our patients so they can get back on the road to recovery.

Physical Therapy

  • Balance and gait training
  • Muscle re-education
  • Therapeutic exercises
  • Prosthetic training for patients with amputations
  • Home modification adjustments
  • Instruct on proper use of assistive devices

Occupational Therapy

  • Re-train for self-care skills such as:
    • Feeding
    • Dressing
    • Bathing
    • Grooming
    • Personal Hygiene
    • Toileting
  • Fabricate splints and casts for positioning, protection, spasticity, and range-of-motion
  • Assess safety conditions in the home and needs for adaptive equipment
  • Improve fine motor coordination
  • Educate the patient about joint protection and energy conservation
  • Prepare patient to resume day-to-day activities
  • Utilize HomeSight™—a specialty program for visual impairments

Speech Therapy

  • Intervene in swallowing disorders using VitalStim® therapy
  • Improve receptive and expressive speech and language skills
  • Enhance cognitive language skills

Eligibility Criteria

  • Age 21 and older
  • Payors:
    • Medicare/Medicaid
      • Patient must be homebound—see section 1, page 5, item 4
      • Requires a skilled need
      • Care ordered by a physician
    • Private Insurance
    • Private Pay

At Home Healthcare is licensed and certified by the Texas Department of Aging and Disability Services (DADS) and the Centers for Medicare and Medicaid Services.
